Application for Clinic Licence

Meaning of Clinic

A clinic is any premises where registered medical practitioners and/or registered dentists practise-

  1. that do not form part of the premises of a hospital, a day procedure centre or an outreach facility;
  2. that are used or intended to be used for providing medical services to patients / carrying out minor medical procedures without lodging;
  3. that do not provide to any person a medical procedure that requires the person's continuous confinement within the facility for more than 12 hours; and
  4. that do not perform any Scheduled Medical Procedures (refer to Section 2 of and Schedule 3 to the PHFO).

A clinic satisfying the meaning of a small practice clinic (SPC) may make a request to the Department of Health (DH) for a letter of exemption.

How to Apply
  1. Read the Code of Practice for Clinic, guidance notes for the application, application form and all relevant forms and documents before application
  2. Fill in the application form through e-Licensing User Guide for e-Licensing
  3. Duly sign the application form and the corresponding declaration forms
  4. Prepare all required supporting document(s)
  5. Submit all the documents listed in (3) and (4) to the Department of Health
  6. Pay the application fee on or before the due date stated on the general demand note. Payment methods are specified on the general demand note

Provisional licence is only applicable to clinics in operation on 30 November 2018 on the premises and the application must be made within the period from 13 October 2025 to 13 April 2026. The application fee must be settled on or before 13 April 2026.

Validity Period of Licence

Full Licence

Under normal circumstances, the licence of a clinic is valid for no more than 5 years.

The licence of a scheduled clinic is valid for no more than 1 year.

Provisional Licence

The provisional licence is valid until one of the following events occurs or the date specified –

  1. the full licence is issued
  2. the application for full licence is refused by DH
  3. the application for full licence is withdrawn, or deemed to be withdrawn by DH
  4. an expiry date of the transitional arrangements appointed by the Secretary of the Health Bureau by notice published in the Gazette

In order to provide sufficient preparation time for the industry, the expiry date of the transitional arrangement will be at least 1 year after the date on which the notice is published.
