Request for Letter of Exemption for Small Practice Clinic

Meaning of Small Practice Clinic
A Small Practice Clinic (SPC) is a clinic that is operated only by registered medical practitioner(s) and/or registered dentist(s). The operation may be in the form of a sole proprietorship, a partnership or a company. Moreover, the following three conditions must also be met -

  1. Number of medical practitioners or dentists;
  2. The right to use the premises; and
  3. Locum arrangement
How to Submit a Request
  1. Read the Guidance Notes for Request for Letter of Exemption for Small Practice Clinics, Guidance Notes for Considering Inappropriateness for a Person to be Issued with a Letter of Exemption for Small Practice Clinic to Carry on Practice in a Clinic Without a Licence, request form and all relevant forms and documents before making a request
  2. Fill in the request form through e-Licensing User Guide for e-Licensing
  3. All operators should duly sign the request form
  4. Prepare all required supporting document(s)
  5. Submit all the documents listed in (3) and (4) through e-Licensing. If you are not able to submit electronically, please submit them by post or contact our office to make an appointment for in-person submission
Fee

Request for letter of exemption for SPC is free of charge.

Validity Period of the Letter of Exemption

The exemption for an exempted clinic will remain valid unless the clinic is relocated, no longer falls under the meaning of an SPC, or the exemption is revoked by the Director of Health.
