Variation of particulars

• Change of name or contact information of DPC, licensee, its authorized partner, authorized representative of company or organisation, or chief medical executive (CME)
• Change of authorized partner, authorized representative of company or organisation, or CME
• Retirement from partnership

Variation of service

• Change in the scale or scope of services as specified in the DPC licence
• Change in the physical location where the services specified in the annex of the DPC licence
• Material change to the premises of the DPC, including but not limited to –
  1. - Additions, alterations and/or improvement to the electrical installations for critical care areas, specialized ventilation systems, or medical gas pipeline systems
  2. - Change in non-movable walls of clinical or clinical supporting services e.g. renovation
  3. - Change in water treatment and distribution system for haemodialysis

1. Read the Code of Practice for Day Procedure Centres, guidance notes for the application, application form and all relevant forms and documents before application
2. Fill in the application form through
3. Duly sign the application form and the corresponding declaration forms (where applicable)
4. Prepare all required supporting document(s)
5. Submit all the documents listed in (3) and (4) to the Department of Health
6. Pay the application fee (if applicable) on or before the due date stated on the general demand note. Payment methods are specified on the general demand note

The validity period would be the same as specified in the existing licence
